 Please keep in mind: ALL PROVIDERS ARE CREDENTIALED WITH THE SAME INSURANCE COMPANIES PRACTICE WIDE (except Dr. Stainback who does not accept United insurance). We are in-network with Anthem, Blue Cross Blue Shield, CIGNA, and United Healthcare. Please note that we do not accept Medicaid, Medicare, or Employee Assistance Program benefits.
